NorthStar Care Connections offers Adult Mental Health Targeted Case Management (AMH-TCM) in MN. Our case managers partner with individuals who experience serious and persistent mental illness (SPMI) to provide recovery-oriented, person-centered support.

What We Do

We help clients build a roadmap toward wellness by working on the following key areas:

  • Conduct a functional assessment to understand strengths, needs, and goals across life domains (mental health, housing, finances, social supports, etc.).
  • Develop an Individual Community Support Plan (ICSP) together with the client, specifying recovery goals and the services needed to reach them.
  • Connect clients to mental and physical health services, including psychiatric or therapeutic support.
  • Assist with accessing social services (e.g., financial assistance, public benefits).
  • Help link to community resources such as housing, education, employment, legal aid, transportation, and recreation.
  • Facilitate access to natural supports (friends, family, community) consistent with the client’s preferences and needs.
  • Coordinate across providers to make sure services are aligned, accessible, and culturally appropriate.
  • Monitor progress toward goals and review or modify the ICSP as needed.
  • Support clients in overcoming barriers to success.
  • Assist with crisis planning to help reduce risk and increase safety.
  • Provide help with obtaining and maintaining safe, stable housing.
  • Assist with financial resources, including applying for Social Security or other benefits.
  • Offer employment or educational support, including connecting to vocational services.
  • Meet with clients at least monthly — in person, over interactive video, or by phone, whichever works best.
  • Build a trusting relationship grounded in hope, resilience, and the individual’s strengths.

Who Is Eligible

Adults (18+) with a serious and persistent mental illness (SPMI), as defined under Minnesota Health Care Programs. Minnesota Department of Human Services Clients must have had a diagnostic assessment (DA) within the past 36 months (or obtain a new one) to confirm eligibility. Services are available regardless of income for those who qualify.
